Description
Effective clinical documentation is a cornerstone of quality care and professional accountability. This session will explore the core principles of accurate, clear, timely, and confidential documentation, with an emphasis on aligning notes with legal, ethical, and billing standards. Participants will learn to identify common documentation pitfalls and implement strategies to avoid them. Through practical guidance and examples, participants will strengthen their ability to write concise, strengths-based, and client-centered notes that clearly reflect clinical interventions and client progress. The session will also cover best practices for documenting risk factors, safety plans, and crisis interventions in ways that meet both clinical and legal expectations. Finally, we’ll examine how high-quality documentation enhances continuity of care, supports interdisciplinary collaboration, and contributes to improved client outcomes.
Learning Outcomes
At the conclusion of this training, you will be able to:

Instructor
Paige Greene, LCSW, holds a bachelor’s degree in social work from California State University, Chico and a master’s degree in Socia Work from California State University, Sacramento. She is licensed in both California and Kentucky. With over 25 years of experience in the mental health field, she’s had the privilege of working with children, adolescents, adults, families and communities. Her background spans clinical practice, administration, and leadership across both the non-profit and public sectors. Currently, Paige is honored to serve as the Director of TRACS (Triage, Referral, Assistance, and Crisis Support) program where she continues her commitment to supporting mental health and well-being. Paige recently relocated from Northern California to Kentucky.
